Health Coverages

 

           Medical Care
Employees choose the medical plan that best meets their needs. Choices include:  a 100% Preferred Provider Organization (PPO) option, a 90/10 Preferred Provider Organization (PPO) option, or a 100% Open Access option.  All medical plans provide major medical and hospitalization and include prescription drug coverage.  You are eligible to participate on the 1st day of the month following your date of hire.

 

           Dental Care
The dental plan helps pay the cost of dental care services for you and your covered family members.  Dental plan benefits include Preventative Care, Basic Services (Fillings, pulling teeth, root canal therapy), Major Services (crowns, bridges, dentures) and Orthodontia. This plan gives you the option of going to the dentist of your choice.
You are eligible to participate on the 1st day of the month following your date of hire.

 

           Vision Care
To help you pay expenses associated with eye care, the vision plan provides coverage for exams, lenses, and frames.  The plan gives you the option of going to the optometrist of your choice. You are eligible to participate on the 1st day of the month following your date of hire.

 

Retirement Plan

 

           Company 401(k)  
To assist you in retirement planning, you are eligible to contribute up to 75% of your pre-tax salary into a 401K account. Mass Mutual administers the plan and offers 17 professionally managed funds in which to invest your money.  You become eligible to participate 60 days plus the 1st day of the month following your date of hire.

 

           Company 401(k) Match

Domino encourages you to develop retirement savings by matching contributions you make to your 401(k) account.  You are eligible to participate 60 days plus the 1st day of the month following your date of hire.
 

           Company Profit Sharing
Depending upon company performance each fiscal year, Domino may elect to make a fiscal year-end Profit Sharing contribution based upon eligible compensation. You are eligible for these contributions after completing 2 years of service with the company. 

 

Life Coverages

 

           Disability Benefits
To provide you with continued income for sickness or injury, Domino provides you with short term and long term disability benefits.  Both short term and long term disability benefits provide you with 66% of your base pay.  You are eligible for this benefit as of your 1st date of hire.

 

   
           Life Insurance
To provide you and your survivors with financial protection in the event of your death or the death of a covered dependent, Domino provides 3 times your base salary in Life insurance. You may, however, elect to purchase supplemental life insurance for yourself through payroll deductions. The benefits received depend upon the coverage in effect when death occurs. You are eligible for this benefit the 1st day of the month following your date of hire.

             Accidental Death and Dismemberment (AD&D) Insurance
AD&D Insurance is designed to pay benefits if you should die or suffer certain physical losses due to an accident. Domino provides 3 times your base salary in AD&D insurance. You may elect to purchase additional AD&D Insurance for yourself through payroll deductions. The benefits received depend upon the coverage in effect when death or loss due to a covered accident occurs. You are eligible for this benefit the 1st day of the month following your date of hire.

 

           LifeBalance™
To provide assistance in the event you or a member of your family encounters work or family issues, LifeBalance is a national resource and referral service available that will provide assistance with parenting, education, elder care, legal, financial, or health and well-being issues.

 

Paid Time Off

 

           Vacation/ Personal/ Sick Days
To provide time away from work for rest and relaxation, Domino provides its employees with vacation days and floating holidays.  If you are a full-time employee with less than five years of service, you accrue 10 days each year. After your fifth year, you accrue 15 days.  After your tenth year, you accrue 20 days. Vacation days accrue monthly from January to October and are pro-rated during your first year of service.  You are eligible for this benefit upon the day of hire. Additionally, Domino provides its employees with 2 floating holidays (personal days) each year. Full time employees accrue monthly, one day for every six months. These days accrue January through December.

Domino provides its employees with 5 sick days each year.  Full time employees, accrue monthly one day for every two months.  These days accrue January through October.

             Holidays
To recognize national holidays, Domino Amjet, Inc. observes nine paid holidays each year for full-time employees.In addition, Domino observes a paid holiday shutdown beginning Christmas Eve and extending to the first working day after the New Year.

           Bereavement
Domino provides
employees the time required to make arrangements, attend services, and grieve if an immediate family member should die. Employees are offered one to three days of paid bereavement.

 

 

Professional/Occupational Development

 

           Tuition Reimbursement
To facilitate individual development, tuition reimbursement is offered to employees to defray the cost of tuition, lab fees and books for approved courses taken at an accredited institution. The maximum benefit is $5,000 annually.  You are eligible for this benefit six months from your date of hire.

 

           Training

Encouraging employees to be both skilled and efficient at their respective positions, Domino provides payment for seminar and conference related fees that are job related or career enhancing.

 

Other Benefits

 

           Service Recognition Program
To recognize long-term commitment and contributions to the company, you will receive a gift card in addtion to a personalized award when you celebrate your  5, 10, 15 and 20 year anniversaries with Domino Amjet, Inc.

 

           Employee Referral Program

Our employee referral program offers employees a payment of $500 when a position is filled by referral.  The referred individual must be employed with Domino for four months in a full time position before payment.

 

           Employee Discretionary Bonus Plan

Employees are essential to the success of our organization.  To recognize your contribution to that success, we have an employee discretionary bonus plan.  Payments are based on company profits and paid out in July and December.  Bonus payout is based on performance and management discretion and is pro-rated during the first six months of employment.  You are eligible to participate upon your date of hire.
